Once you have submitted the form  below, you will have the option to book a complimentary discovery call with your preferred Allied Health Professional. This occurs prior to an initial consult to ensure alignment of goals, funding and consider planning for future services. If you do not wish to book a discovery call, please indicate this on the form and we will be in touch to book an initial consult.
